The manufacturer Varian Medical Systems Inc. informs per customer technical bulletin about a single incident during use of a flexible implant tube with blind end used for afterloading therapy. Because of an material deficiency a marker wire could be pushed through the closed end. The manufacturer asks the customers that the marker wires should be carefully inserted.
