Nucletron B.V. has issued a Field Safety Notice concerning a problem regarding the use of Oncentra Visir of the versions Oncentra® Record/Visir v2.0 und v2.1 and their service packs. The consequences of this error is that beams that have reached the planned number of fractions may still be treated, resulting in too many fractions delivered and an overdose to the patient.
