Olympus has issued a Field Safety Notice about safety instructions for Lens Cleaning Sheath MAJ-1537 useable with LTF-VH (Laparo-Thoraco Videoscope). Before each procedure it must be checked that there are no irregularities such as cuts on the tube of the sheath. Sheaths with any irregularities should be disposed.
